Mission Statement
Hazelhill Family Practice, works as a multidisciplinary team, which aims to provide a range of health care services that are designed to provide a comprehensive health care to our practise population. Our health care services range from screening people at risk, and when appropriate, diagnosing and treating medical conditions, followed by evaluation and follow up.
Our service further provides Health Promotion, lifestyle advice, awareness to reduce the risk of disease, diagnosis treatment and follow up. We afford the first level of contact for our community that is fully accessible by self-referral, thus providing a comprehensive and approachable forum to individuals in the community. Our emphasis is on a holistic approach to health care for all in the community setting.

Confidentiality is extremely important at Hazelhill Family Practice. All information is confidential and will only be released to a third party with your written authority or by order. All staff sign confidentiality agreements. We ensure all patient consultation medical records remain strictly confidential.
